East Boston Neighborhood Health Center Corporation will use NMTC allocation to finance land aquisition and the new construction of 15,000 SF of space for a PACE (Program of All-Inclusive Care for the Elderly) Day Center. The mission of the PACE program is to serve low-income, nursing home-eligible seniors with comprehensive health care and support services in their community, thereby allowing them to live independently outside of nursing homes. EBNHC is one of the leading health care providers in Boston. It provides 300,000+ patient-visits a year to the diverse and low-income populations of East Boston, as well as Boston's inner northern suburbs of Winthrop, Revere, Everett, Chelsea, and Malden. The project will create a number of benefits for EBNHC, its clients, and the larger community.
NORTHERN NEVADA HOPES

Northern Nevada HOPES is a community health center that provides integrated medical and wellness services to both insured and uninsured patients. With this development, the health center is constructing a 38,000 SF clinic on its current site that will allow it to broaden its services and greatly expand patients served. The new 3-story building will provide a primary care clinic, pharmacy, offices for client consultation, behavioral health, social services, and admin space for the sponsor. The development will allow HOPES to expand the number of providers from 5 to 16 over the next three years and to have a full pharmacy.
BROCKTON NEIGHBORHOOD HEALTH CENTER

Brockton Neighborhood Health Center is a Federally Qualified Health Center committed to providing care that is responsive to community health needs and is linguistically, culturally and financially accessible. This new 13,600 SF community health center will expand BNHCâs local services to help meet the needs of this medically underserved neighborhood in Brockton, MA. The new building will provide 27 exam rooms and accommodate 40,000 patient visits annually. The clinic will also have an active nutrition program that will be operated in partnership with Vicenteâs Supermarket, located next door. The health center will have a Demonstration/Teaching Kitchen and patients will receive instruction on proper cooking techniques and how to incorporate fresh seafood, vegetables and fruit.
HIGH POINT TREATMENT CENTER

High Point Treatment Center launched in 1997 with the commitment to excellence in serving individuals, families, and communities impacted by addiction, mental illness, domestic violence, and/or homelessness by helping them achieve their greatest potential in recovery. This new facility in Marlborough, MA transformed the former St. Lukeâs Hospital into a 72-bed, state of the art psychiatric facility. The facility features a primary care clinic, mental health counseling, youth counseling, and job search assistance. This development works towards filling the gap in psychiatric services in Massachusetts that had been forcing HPTC to turn away patients at its existing facilities due to overcapacity.
JOSEPH M. SMITH COMMUNITY HEALTH CENTER

Joseph M. Smith Community Health Center opened its doors in 1974 and since then has been dedicated to improving the health and well-being of the communities of Allston, Brighton, Waltham and surrounding areas by providing quality, comprehensive, coordinated care. Despite its four Allston-Brighton locations JMSCHC was operating at capacity and unable to meet the growing new patient and community need for care. In response, JMSCHC opened a new, 48,000 SF building that was able to centralize services and staff, provides savings of over $300,000 in annual rent, offers 3x more space for care and community classes and events.

The East Boston Neighborhood Health Center is one of the leading health care providers in Boston, and among the largest community health centers in the country. This new, 48,000 SF medical clinic building located in East Boston is a 4-story healthcare space for serving patients as well as administrative office space and training space for EBNHC. The project also contains a vision center, ground floor retail space, and a dental clinic. At full capacity, the Center will be able to accommodate approximately 100,000 patient visits per year and another 230,000 people per year coming as family members of patients or training class participants.
